What to look for in the event of a head injury.

IT’S SPRING!! We all know what that means, nicer weather and going outside to play. Kids have started getting ready for the soccer season and baseball and softball are not far behind. Whether playing a sport or just getting outside to run around, injuries are bound to occur. One particular issue that is being focused on are concussions. The CDC has initiated a program called “Heads up to Schools: Know your concussion ABC’s”.

To start off, what is a concussion? A concussion, put simply, is a brain injury that interferes with the way the brain functions. Approximately 65% of concussions occur in kids from 5 – 18 years of age. There are a few reasons for this, one being that the brain is still developing at these ages which makes it more sensitive to injury. Another reason is the immaturity of the neck and shoulder musculature which means less support of the head. Remember concussions occur due to not only hitting your head directly on objects but also indirectly. A good example of a brain injury that occurs without direct blunt force to the head is shaken baby syndrome. In this instance the brain moves back and forth within the skull causing injury to the brain that can result in bleeding, swelling and bruising of the brain. Ultimately, even if your child has not had a direct blow to the head, if they are displaying signs and symptoms of a concussion, further evaluation should be completed.

Now back to the ABC’s! A= Assess the situation. B= Be alert for the signs/symptoms. C= Contact a healthcare provider. Assess the situation, how did it happen? Did the child get hit directly in the head? Or did they receive a hit to the body that ultimately caused rapid movement of the head? If at any point in time there is concern of a neck injury DO NOT move the individual. Keep them as immobile as possible until EMS arrives.

Be alert for signs and symptoms. What should we look for? There are a number of things to look for to include but are not limited to the following:

- Headache
- Dizziness
- Balance disruption such as unsteady walking
- Sensitivity to light or sound
- Confusion or feeling mentally “foggy”
- Delayed verbal and motor responses
- Slurred or incoherent speech
- Irritability
- Excessive drowsiness
- Loss of consciousness ( which only occurs with 10% of concussions)
- Unusual behavior

In the event that there is a loss of consciousness for greater than 30 seconds, increase in headache or confusion, repeated vomiting, weakness/numbness to the extremities, slurred speech, cervical pain with tenderness and loss of range of motion is noted, then immediate evaluation is needed in the ER. Please remember that signs and symptoms may not appear right away.

Contact a healthcare provider. Ultimately, as a parent you know your child better than anyone else. If there is anything that seems to be out of the norm for your child, have them evaluated. Generally speaking, anyone who has a concussion should not return to physical activity while they are still experiencing signs and symptoms of a concussion nor should they return to play the same day of the injury. Your doctor will be able to guide you in your child’s return to activities.

I hope that you have found this information helpful. The CDC website has fact sheets for parents, teachers, and coaches and also has other information that you may be of interest to you. http://www.cdc.gov/Features/ConcussionABCs/

Now get outside and enjoy spring!

We recently heard about an elderly woman in a nursing facility that collapsed and needed CPR. Unfortunately, when the 911 operator asked one of the nurses to begin CPR they said no (because of facility rules). The 911 operator desperately asked for anyone to give CPR and either nobody would or nobody knew how.

This is why it’s so important to know CPR. You literally don’t know when it might be needed and more than likely it will be when you least expect it.

Great News: AIR met with ODJFS early January to find out where the rules for allowing all online learning for Type B’s stood. We received clarification on the existing rules. AIR is able to provide Type B’s 3 hours of electronic media per year. That means you can take the refresher Child Abuse or refresher Communicable Disease. You may also take the Initial Child Abuse or Communicable Disease but may only count 3 of the hours towards continuing education. The First Aid and CPR must remain in the face-to-face format for now. There is a chance that changes to allow all online learning will go into effect late spring. We will keep you posted.
